Cellulite is usually accompanied by an excess of fatty tissue but this is not always the case. Often our slim, trim even underweight women will exhibit orange peel skin. Cellulite does not occur in the intramuscularly fat tissue, but in the layer of fat just below the skin, the subcutaneous or adipose fat layer. Even dramatically underweight persons such as Anorexia Nervosa have a thin fat layer under their skin. This adipose fat layer acts as an insulating blanket over our bodies preventing loss of body heat. Only in recent years has this adipose fat layer been recognized as important as other major organs. STAGES OF CELLULITE First stage - flab There are few apparent signs, in some cases the skin might feel like dough but there is no pain. The microscopic examination shows oedema (watery fluid accumulations) and alteration of the fatty cell shapes and of the cell units. This is a indication of the beginning of the metabolic alteration in the subcutaneous fat layer. Second stage - flab (the beginning of orange peel skin) The skin feels doughy, indicating oedema of the subcutaneous fat layer and in some cases, pressure with the finger tips leaves marks which represent oedema of the dermis level. Third stage - "orange peel" more advanced Pinching and manipulation of the skin causes dull pain, modules are not detected under the microscope, micro nodules are seen. Fourth stage - "orange peel" and nodules are visible Painful skin, (sometimes untouched) and micro nodules or plaques with or without oedema. The microscope reveals encapsulated nodules and hardening of the connective tissue. The cellular activity in the adipose fat layer is very intense and sophisticated, Its relationship with blood and lymph circulation is of the utmost importance for general metabolism. Subcutaneous or adipose fatty tissue equals problem area. Where does the intercellular waste go? It is absorbed by the lymphatic system. The system has an abundant blood supply provided by a complex network of capillaries. Micro arterial capillaries bring in oxygen, venous capillaries carry away depleted blood, and lymphatic vessels take the lymph to be added to the venous blood circulation and excreted through the kidney and urinary tract. However, the fat does not sit still until needed, as previously believed, but rather goes through a very dynamic and ongoing process.
